  • Your contact information
  • What program, team or employee your complaint relates to 
  • The details of your complaint
  • What action you would like us to take

If you have a complaint about a government agency or service other than the Child Death Review Board or QFCC employee, please make your complaint with the department that provided the service. If the issue cannot be resolved with the department, we recommend you contact the Queensland Ombudsman.
